Radio Terapia y Preliminares del Trasplante

 

 Los Doctores, Jefes de su Departamento-: Arnold S. Freedman (Hematólogo-Oncólogo), Robert J. Soiffer (Trasplantes de Células Fuente) y Peter M. Mauch (Radioterapia-Oncológica) del Dana-Farber Cancer Institute y el Brigham and Women's Hospital, ambas instituciones hermanas afiliadas a Harvard Medicine.

El Dr. Peter Mauch, M.D.,  se ubica en el piso L2 por los elevadores denominados Amory, del Brigham and Women’s Hospital, esto es en el segundo sótano. El Departamento de Radiación Oncológica es grande, cuenta con dos maquinas de radiación, con su propia sala cada una y un personal altamente calificado. Cuenta por lo menos 5 enfermeras especializadas y 5 técnicos. Aparte se encuentran una serie de Doctores, el asistente del Dr. Mauch y la Enfermera en Jefe, Lynn Lopes, R.N.  Cuatro o cinco forman el personal administrativo, un área para enfermos en cama y una amplia sala de espera, que ya parece chica por el movimiento constante de pacientes ambulatorios en espera de ser llamados para su radiación diaria.

Marilú empezó a recibir Radioterapia el 15 de Enero pasado y terminará el 10 de Febrero próximo, 18 sesiones. Se ha aplicado diariamente, excepto en los fines de semana, estos dos días están pensados para que la células sanas se rehabiliten. Cada cinco aplicaciones le toman radiografías, con esto ven cómo van y rectifican sus mediciones si fuera necesario, para la aplicación exacta del radio. La aplicación diaria es de dos a tres segundos, después de acomodar los aparatos, que radian por arriba y por debajo de la plancha de reposo, para no dañar la piel en un solo lugar, si estuviera expuesta a radiación por más segundos de los que la exponen. Nos tardamos más en viajar al hospital y en la sala de espera cuando se atrasan, que el tratamiento  en si…

El 20 de Enero, pacientes, enfermeras, técnicos, personal administrativo, Doctores,  todos aglomerados en la sala de espera; viendo la toma de posesión del ahora Presidente Obama. Todos altamente emocionados, veían en la TV, como se sucedía el cambio de poder, y ahí todos apoyaban al Sr. Obama. Había mucha emoción y a veces me confundía ya que no solo había emoción por el Presidente entrante Obama, sino que también  había alegría porque George W. Bush, partía. Las dos emociones hacían del ambiente algo muy especial. Todos ahí estábamos encerrados en esos sótanos pero el entusiasmo era cuantioso, alegre ¡y hasta los enfermos no parecían estarlo!

Así estaba toda la Nación Americana. Las muestras de entusiasmo y cariño hacia la familia Obama, fueron espectaculares en todas partes y de parte de todos, de todas las razas y de todas las edades. No hubo falta de respeto hacia los Bush, solo el gusto de que ya se iban. Hubo orden y todo el ambiente generaba confianza. Las minorías de color aún mas entusiasmadas ¡Eran testigos de la realización el sueño del Dr. King!

Al terminar la ceremonia de Juramento, pasaron a Marilú, le dieron su tratamiento de dos segundos y partimos al Departamento para seguir viendo los eventos en Washington. Fue impresionante  como el suceso elevó la esperanza de una enorme mayoría, dentro de la gran turbulencia que en todos los frentes ocasionó la administración saliente.

Hemos platicado con el Dr. Mauch, Lynn y el Dr. Asistente Directo tres veces, cuando revisamos -paso por paso- el progreso del tratamiento y sus resultados, también se repasan todas las posibles afectaciones que todavía se pudieran suceder. Marilú no ha sufrido de ninguna consecuencia secundaria, hasta ahora el domingo 1 de Febrero. Sin embargo ya supimos que la gripe o catarro, que la venía afectando, bien pudiera ser consecuencia de la quimioterapia, antes de la radiación que le bajo sus defensas.

El Dr.Mauch, está muy orgulloso de que sus hijos hablan español, aprendido de la institutriz que trabaja con su familia en su casa. La enfermera Lynn Lopes lleva muy en orden todo lo que se sucede en el Departamento de Radiación Oncológica del BWH/DFCI, son muy eficientes y sus sistemas computarizados de primera.

Todo el expediente Médico de Marilú está concentrado de manera electrónica y cualquier persona autorizada dentro de las instituciones Médicas afiliadas a Harvard, pueden tener acceso a ellos. Esto facilita todo, inclusive las medicinas recetadas llegan a la farmacia en órdenes electrónicas para ser surtidas. El seguro está informado de cada gasto exactamente en el momento que se sucede.

Y esta semana que termina, empezaron los exámenes necesarios y esperados para el trasplante. Le checaron que la dentadura estuviera en perfecto estado, extrajeron 14 tubos de sangre para diferentes pruebas, aplicaron medicina nuclear para ver como se encuentra el corazón, tomaron electrocardiogramas, exámenes de los pulmones. ¡Se confirmo que Marilú si sopla! Muchos de estos miden la fuerza del aliento soplando con ganas.  Y le rasparon para ver su reacción a la TB. La medicina nuclear fue interesante, sacan sangre y la mezclan con un elemento radioactivo que vuelven a meter, para después ver como late el corazón y medir sus latidos, con equipo especial que gracias a esa sangre radioactiva puede hacerlo. Todos los días miden la presión arterial…  

El día 19 de Febrero internan a Marilú, para una semana de quimioterapia y recibir las células madre de Tere el 27 o 28 de Febrero, estos dos días son los días clave. La colección de las células madre de Tere será antes el 25 o 26 de Febrero. Tere llega a Boston el día 11 para empezar su chequeo médico el 13 de Febrero, a ella también la checan, obvio. Todo empieza a darse forma y a tomar su lugar. La semana entrante el día 4 y después también, tenemos varias citas de instrucción con los Doctores y su equipo especializado.

¡A eso vinimos! Y Marilú esta lista para salir adelante con éxito –tal y como ha pasado todas estas y otras pruebas- Tenemos confianza, primordialmente radiada por la propia Marilú, por la capacidad del Equipo Médico, por los adelantos que vivimos de la ciencia médica,  por el apoyo que nos han dado nuestros hijos y nietos en primera instancia y por todas las muestras de cariño que hemos recibido de familiares y de nuestros queridos amigos.  Avanzamos y seguimos…

English General Report on our Boston Stay

A General Report on our Boston Stay (For our English Speaking Friends)–January 2009-

Many of our English speaking friends have requested more postings in English on this Blog. We are gladly conceding to their wishes and desires. In this present posting there are no links, or videos or pictures, most of them except one or two throughout the Blog, are in English anyhow and the pictures posted speak for themselves. So please bear with us those phrases that are wrongly constructed, it is due to our Spanish background, the odd thing, maybe due to old age, is that many of our phrases in Spanish carry an English construction; sorry it is an inevitable phenomenon!

The underlying reason for our Boston stay is, as you know, that Marilu was diagnosed with a Mantel Cell Non-Hodgkin’s Indolent Lymphoma about 30 months ago. She undertook the first phase of chemotherapy in Mexico City. She was infused with the CHOP-R chemo. Everything went well and she was declared in total remission by Dr. Arnold Freedman Head Hematologist of the highly reputed Dana-Farber Cancer Institute a Harvard Medicine University Affiliate, on one side and on the other by Dr. Alberto Villalobos Hematologist of the ABC Medical Center in Mexico City. Both Doctors worked in coordination treating Marilu, and were very enthusiastic by the excellent results and general health condition of Marilu throughout the treatment.

On a latter visit in April 2008, with our Primary Care Physician in Boston, Dr. Ronda Rockett of Wellesley Massachusetts ordered a colonoscopy for both of us, which is a routine procedure for patients our age. Victor was surprised to be found in good shape, for years he had suffered stress colitis, which left no lasting effect; but the Gastroenterologists and Pathologists of the Wellesley-Newton Hospital, re-discovered lymphatic cells in Marilu’s intestine. This meant that the cancer was back or perhaps that the CT Scans performed originally were not as accurate as a colonoscopy. We were shaken by this second rude awakening…

The new facts forced us to rethink our strategy. The Chemotherapy infused in Mexico could not have been better applied. We were gratified by the diligent work performed by Dr. Alberto Villalobos at the ABC Medical Center in Mexico City, who kept the Dana-Farber team in Boston in the loop. Dr. Villalobos, and generally all cancer medicine in Mexico follows closely those experiences in South Western USA Medical Centers; Baylor, the Anderson Clinic in Houston, Texas, and very recently the cancer joint venture clinic, to be opened in Mexico City, of the ABC and the Methodist International Network. So we had choices to make.

One approach was to wait and see how the returned indolent lymphoma developed. The other opinion, was to act now, when Marilu is in top condition and in excellent general health, not to wait for any further developments that nobody knew if and how these would show up. One consideration was also her age and the health and age of the potential compatible donors and less important morally but in need of reflection was the financing involved.

We are inevitably under the influence of Andy’s miracle. A fantastic successful scientific achievement of the Harvard University related Hospitals. The Children’s Hospital Boston diagnostic and care capabilities, the Dana-Farber stem cell transplant teams that perhaps have the most advanced experience in the world in the field. The state of the art medical installations in the Longwood Harvard Medicine Campus in Boston persuaded our choice.

On the other hand we knew that the ABC Medical Center –a general hospital- was the best we could get in Mexico, under the umbrella of the mediocre insurance services backing up our case. Carmina’s -Marilu’s sister- experience at the Mexican Nutritional National Institute and other hospitals were of no encouragement either.

While Dr. Alberto Villalobos, had all our confidence, he stood really alone against teams of experts at the Dana-Farber Cancer Institute in Boston Massachusetts.  The different philosophies in the application of cancer medicine and perhaps in every other medical field had a lot of weight in our decision process.  We felt with better insurance cover in the US and had experienced that the general organizational systems in Boston were far much better than those in Mexico.

We had Andres our son and Ana Paulina his wife, established in Boston that proved to be of invaluable help. Victor Antonio and Veronica were also in favor of their mother receiving the best possible treatment available in the world, within our reach.

We arrived in Boston on August 10^th 2008. Victor our son is carrying alone the full weight of the business in his shoulders. We pray that the present difficult world-wide economic and business conditions do not hamper his success; but somehow have the opposite certainty, a deeply rooted knowledge that he will come out better with the crisis than without it!  

Veronica in a moment of inspired decision, convinced Juan her husband, to take the sabbatical they were contemplating for some time and started organizing their affairs to carry out this somewhat long desired wish in Boston.

Andres helped and found them a house to rent in Sudbury, MA, near him. This house then provided the assurance for the children to enter Sudbury, MA, excellent public schools. Juan and Veronica travelled to Sudbury and arranged everything in a week including furniture they purchased in IKEA and assembled immediately, for their arrival together with their four children…

In the meantime Andres rented our Apartment in the Arborpoint complex in the Woodland Station in Auburndale, one of the 13 Villages of Newton, MA. As a welcoming present, Andres was kind enough to buy our beds in IKEA as well, during one of Veronica’s many visits to the store. So we were all set!

Our first week in Boston was spent furnishing our apartment. Andres assembled all of our IKEA purchased furniture, for which we thank him profusely. We informed the Dana-Farber Cancer Institute of our arrival but our appointments started until September once Dr. Freedman and Dr. Soiffer were back from vacations. Therefore we had time to settle in…

Andres and Paulina have opened many doors and paved many roads for us. Having them in Boston has made our life so much easier and better. Veronica’s move with her family has brought extraordinary joy to our stay. For the first time –not on vacations- we have our six grandchildren together. In April Ana Paulina will give birth to their third child our seventh grandchild.  By that time, Marilu will be clean or well on her way to remission from her lymphoma and ready to receive with great joy our seventh! Victor comes and goes between Mexico City and Boston, so we have not had time to miss him.

We have been able to take advantage of the many things Boston has to offer. We have attended concerts; we have been at the Boston Ballet. We have gone to the circus, the theatre; we have visited museums, and have been in outdoor events like canoeing in the Charles River, skiing, bicycling, and picnicking. We have visited many restaurants. We have been shopping for Christmas. We have gone to the movies more than ever. We have visited some of the most important centers of higher learning in the region and we still have a lot to see and do!

We have been treated at the facilities of the Dana-Farber Cancer Institute, The Brigham and Woman’s Hospital in the Longwood Campus at Harvard Medical and The Wellesley-Newton Hospital across the street from our apartment.

It has been very gratifying to have so many friends calling us and emailing us. We have not felt lonely at any one moment. Being in touch, yet far away, is a modern present that we have been happy to receive.

We have been fortunate in being witness in the US during this 2008/2009 crisis, but more importantly during the historic Obama campaign and the transition from the Bush administration to a new era of inevitable change. Having survived past economic crisis in Mexico, the main two in 1982 and 1994, we know that many of the established things will necessarily have to change. The measures taken by the US government so far are very similar if not some identical, to those taken in Mexico during those mentioned years, the scale is different and the underlying main problem has mainly been excess government spending and mismanagement of public affairs of an outgoing administration that lost control of the situation.

Some difficult measures may still have to be taken; there has even been talk of nationalizing the American Banking Sector to end the banking crisis. Selling of banking and other corporations to foreign firms will gradually erode sovereignty in America; American firm’s decisions will be taken in foreign capitals without consideration to local needs. A new “Toxic Asset Receptor” organization will be created, so that banks are able to operate without the weight of nonperforming assets, leaving the recovery of whatever value remains and legal problems to the new organization. The Dollar will have to devalue steeply once other economies recuperate, interest rates will raise and inflation will be of two digits for some years to come. It will not happen now, as competition with other damaged economies is not immediate. The urgency is to get the world economies going!

The present war on terror on two fronts must come to an end soon. The US economy cannot afford to continue unless the USA figures out a way to receive compensation for its actions from the defended nations, compensations that those nations could afford. Can then Iraq and/or Afghanistan repay the US? Can the allied Arab countries pay for their share in the cost of having been relieved of Sadam Hussein? Can allied nations instead of contributing with men and arms, help pay for the burden? At this time these allies cannot pay for the burden but most likely will not and refuse any monetary collaboration, it would perhaps be considered an insult to attempt such a thing!

Much more can be said, once we get a feeling of the choices the present administration is most likely to make. The US must become competitive again; hopefully its work force will be able to understand that many of the privileges they had won in the past, are no longer viable and will be gracious enough to roll up their sleeves and put things right, otherwise anyhow, all will collapse without giving them a chance to regain their advantaged position in the future. Once more the regained strength of the local consumer market will show which way out of the crisis to take! Therefore local market strength must be enhanced and credit must be restored!

Now is one of the most interesting moments in history to witness and participate, while we tackle the other -at fearful moments- seemingly impossible task of recuperating health. We are the recipients of many good wishes and prayers. We are also under the influence of unconditional love from our children and grandchildren. We feel truly blessed and are thankful for being allowed to perform under the circumstances with the best of ourselves. We will be back to what we deserve, a life full of possibilities! After all, that is the primary reason why Marilu married Victor, for his possibilities…

Radiation Therapy is the use of radiation to treat cancer and other diseases. There are different types of radiation. One that you may know about is x-rays. If you’ve ever had an x-ray of the chest or any other body part, you have had some radiation. This same type of radiation, but in much higher doses, can also be used to treat some types of cancer.

La Terapia de Radiación es el uso de radiación para tratar el cáncer y otras enfermedades. Hay diferentes tipos de radiación. Uno que se conoce es el de los rayos-x. Si ha recibido rayos-x en el pecho o en cualquier otra parte del cuerpo, ya ha recibido radiación. Este mismo tipo de radiación, pero en dosis mas altas, se puede usar para tratar algunos tipos de cáncer.

How does it work? / ¿Como trabaja?

The body is made up of millions of normal healthy cells. When cancer occurs, something causes a normal cell to become a cancer cell. This cancer cell then grows and makes more cancer cells until a tumor is formed. This tumor can grow and cause symptoms. If the cancer is not treated, it can form more tumors, which can affect the rest of your body.

El cuerpo está formado de millones de células sanas y normales. Cuando se sucede el cáncer, algo causa que una célula normal se convierta en una célula cancerosa. Esta célula de cáncer crece y hace mas células cancerosas hasta que se forma un tumor. Este tumor crece y causa síntomas. Si el cáncer no se trata, puede formar mas tumores, que afectan el resto del cuerpo.

The purpose of radiation therapy is to kill the cancer cells. Radiation therapy uses special equipment that sends high doses of radiation to the cancer cells or tumor. This prevents the cells from growing and making more cancer cells. Radiation therapy can also affect normal cells near the tumor. But normal cells can repair themselves and cancer cells cannot.

El propósito de la terapia de radiación es el de matar a las células cancerosas. La terapia de radiación utiliza equipo especial que envía altas dosis de radiación a las células cancerosas o al tumor. Esto impide el crecimiento de las células y que creen mas células cancerosas. La radiación puede afectar a las células sanas cerca del tumor. Pero las células normales se reparan por si mismas, mientras que las cancerosas ya no pueden.

Sometimes, radiation therapy is the only treatment a patient needs. At other times, it is only one of the patient’s treatments.

A veces, la radiación es el único tratamiento que necesita el paciente. Otras veces, es uno de los tratamientos del paciente.

Radiation therapy differs from chemotherapy. Radiation therapy treats just the tumor. Chemotherapy uses drugs to treat the entire body. So Chemotherapy might be used if a person had cancer throughout their body. Radiation is a local treatment. This means it affects only the part of the body being treated.

La radiación es diferente a la quimioterapia. La radiación trata solo al tumor. La quimioterapia usa químicos para tratar el cuerpo entero. La quimioterapia se puede usar cuando la persona tiene cáncer por todo su cuerpo. La radiación es tratamiento local. Esto quiere decir que afecta solo la parte del cuerpo bajo tratamiento.

The chemotherapy applied to Marilu has eradicated the lymphoma throughout her body, except that located in her intestine. This is the reason for the radiation therapy that is being applied now. Remember the lymphoma was discovered in an under arm node. The stubborn cancer cells located in her intestine have resisted three different types of chemotherapy; radiation therapy will prevent these cells in forming a tumor that would require a more difficult treatment and this will allow to carry out the planned stem cell transplant permitting the new mother cells to graft without complications of any sort. The radiation therapy is every day (rest during weekends) for three and a half weeks.

La quimioterapia aplicada a Marilú erradico el linfoma en todo su cuerpo, excepto a aquel localizado en su intestino. Esta es la razón por la cual se está aplicando ahora terapia de radiación. Acordemos que el linfoma se descubrió en unos de los ganglios en la axila.  Las tercas células linfáticas localizadas en su intestino han resistido tres diferentes quimioterapias; la radiación impedirá que estas células formen un tumor que requeriría tratamientos mas difíciles y esto permitirá el llevar a cabo el planeado trasplante de células madre permitiendo que estas se homologuen sin complicaciones de ninguna especie. La radiación es diaria (excepto durante fines de semana) por tres semanas y media.    

Many patients have no side effects at all, while others have some side effects. The most common side effects are:

• Fatigue (feeling tired)
• Skin changes
• Loss of appetite

Muchos pacientes no sufren de efectos secundarios, mientras que otros si tienen efectos secundarios. Los efectos secundarios mas comunes son:

·         Fatiga (sintiendo cansancio)

·         Cambios en la piel

·         Perdida de apetito

 

We received a complete Guide with all suggestions from the Radiology team at the Brigham and Women’s Hospital. The team is headed by the respected Dr. Peter M. Mauch, M.D., who is in daily contact with his colleagues at the Dana-Farber Cancer Institute, both Harvard Medical University Affiliates.

De memoria re-establecimos el Blog. Nos quedamos con la duda si ya lo habíamos completado en su totalidad. Estamos seguros de un tema faltante, el que se refería a nuestra visita al Ballet de Boston, y el otro, creemos era en el que tratábamos sobre la localización de nuestro departamento enfrente de la estación del metro de Woodland en Auburndale, una de las 13 villas de Newton, donde vivimos en Boston. Con estos dos casos, ahora si ya sentimos, que el Blog original está totalmente recuperado.

BOSTON BALLET


El Ballet de Boston, una compañía profesional de danza, dentro de la región de Nueva Inglaterra, la segunda organización de tamaño en el ramo, dedicada a los más altos estándares de la danza, entrenamiento y educación. La Compañía, ofrece durante la presente temporada de ocho meses, trabajos clásicos y contemporáneos. Alcanza a una audiencia de todo el Estado de Massachusetts y en la región. Apoya la misión institucional de fomentar valores de liderazgo, innovación y asociación. El Centro de Educación de la Danza del Ballet de Boston, sirve a un imperativo de entrenamiento de bailarines de todas las edades y provee un acceso general al arte de la danza.


El Boston Ballet, fue fundado en 1963 por Virginia Williams, estableciéndose como la primera compañía de ballet en Nueva Inglaterra. Desde entonces, asombra al mundo artístico por su habilidad en el fomento de talentos en esta rama del arte.
Fuimos con las niñas solamente, a ver la producción en Ballet del cuento de la Cenicienta. El Boston Globe, describe la función como entretenida, cromática y amistosa familiarmente.




El Ballet de Boston lanzó su temporada 2008-2009, con este cuento de todas las épocas, contado ahora de una manera nueva y fresca por el afamado coreógrafo James Kudelka. Es el relato mágico de la convicción de una doncella sobre la felicidad más allá de la represión y el poder de transformación y la fuerza del amor. Kudelka desarrolla el tema de manera única dentro de una escenografía Art-Deco y atuendos de los 1920’s, el cuento de nuevo re-imaginado para el deleite de una audiencia contemporánea. Una velada que fascina a las audiencias con su fina elegancia y calabazas que aparecen de los lugares más sorpresivos… Se les hicieron cortas a las niñas las dos horas y 25 minutos con dos intermedios. Carolina fascinada, Sofía, quería aplicar sus lecciones de ballet y la grande Isabel también encantada.
